Check-in for all teams will tak place at the concession stand in Civic Park on Friday evening, September 17th. Check-in will begin at 6:00 PM and end at 8:30 PM. Team representatives should have the following items with them in order to check-in a team:

ITEMS THAT WILL BE KEPT:

A copy (not the original) of your Team Roster, approved by the team's state association.

If your team has guest players, a Guest Player Roster, approved by the team's state association.

A travel permit, for teams outside of Ohio South

ITEMS THAT WILL BE INSPECTED AND RETURNED:

A combined Liability/Medical Release Form, signed by an adult for each participant, is preferred. If such a release form is not available, the USYSA Player Membership Form for the current year that has BOTH the medical treatment and liability sections completed is acceptable.

Player Passes with photo (required for all participants, including guest players).

Player Passes will be cross-checked against the Team Roster and the Liability/Release Form. Player Passes must be presented to the referee at each game.
